Thinking about saving some time?  We appreciate your time so we have provided documents
for you to fill out at home or if you get a break at the office.  Please choose the necessary
forms that meet your childrens' needs  and print them out, or save yourself some writing
and modify your information using our PARENT PORTAL. You will need Adobe Reader
to view all documents below, and there is a link to download at the bottom of the page.

Note: Registration Fees for all students are $60 dollars and
students in grades 1-8 have a $10 technology lab fee

PARENT PORTAL

We are proud to announce that you can now save valuable time by using the PARENT PORTAL to modify much of the information that we have on file for your family.  These changes you make on the portal are reviewed and responded to by our district secretaries.   If the district requires further information, you can expect an electronic response, but we hope to utilize this tool to save you valuable time prior to the open registration.  If you have not signed up for the Parent Portal, please click the above link and view the documents to learn more.  You may modify your information on the portal by clicking on the family data button within the portal after you login, just click the edit button and make your desired changes.  Thanks for utilizing this wonderful tool!

Note: The changes made on the Parent Portal have to be authorized by district staff and we will still require a returning student registration form found below, but this will save you time when we ask you for residency, work information, and emergency contact information at registration time.


Please read the following situation that best suits you:

If this is the FIRST TIME your child has ever attended a school
IN ILLINOIS, then please fill out the following forms:
  

  1. New Registration
  2. Verification of Residency
  3. Department of Human Services Health Exam
  4. Dental Examination (ONLY IF ATTENDING KINDERGARTEN, 2ND, OR 6TH GRADE)
  5. Eye Examination
  6. School Athletic Physical (ONLY IF INTENDING TO PLAY SPORTS)

If you are transferring a child or children from a school
IN ILLINOIS then your previous school should
have all the health information depending on the grade
specific information listed above, and we would only require the following:
  

  1. New Registration
  2. Verification of Residency
  3. Request for Records


If your child attended District 12 last year,
then please fill out the following forms:
    

  1. New Student Registration Form (ALL GRADES)
  2. Verification of Residency (ONLY IF ADDRESS HAS CHANGED ALL GRADES)
  3. Department of Human Services Health Exam (ONLY IF IN 6TH GRADE)
  4. Dental Examination (ONLY IF ATTENDING KINDERGARTEN, 2ND, or 6TH GRADE)

FACILITY USAGE

If you or your organization wish to utilize the District 12 facilities
for an event, you will need to submit a usage form to the unit office available below:

Brighten A Child's Life January-26-2012
During the months of February and March Big Brothers Big Sisters is planning a community wide awareness campaign in Clinton County called "Brighten A Child's Life".
